Emergency Preparedness “Like Facility” Roster Due May 29

Important: Skilled nursing facilities and assisted living communities must complete the Emergency Preparedness Temporary Shelter “Like Facility” Survey by May 29 to be listed on this year's roster.

The Center for Medicare and Medicaid Services (CMS) Emergency Preparedness Requirements for Medicare and Medicaid Participating Providers and Suppliers requires healthcare providers to establish multiple, pre-determined evacuation locations with “like facilities" that have suitable space, utilities, security, and sanitary facilities for individuals receiving care, with at least one facility being 50 miles away. 

LeadingAge Wisconsin helps member skilled nursing facilities and assisted living communities (hereinafter, "facilities") meet this requirement. Through this survey, we identify members who are willing to enter into a memorandum of understanding (MOU) with fellow providers as a “like facility" evacuation center. Facilities that complete the survey can access the Excel spreadsheet listing providers who responded.

If you have not yet completed the survey, please do so before the deadline of May 29, 2026, in order to be listed on this year’s like-facility transfer roster.
